Audit
Evidence for audit
Producing the pack alongside the work rather than reconstructing it later.
Evidence assembled after the fact is weaker evidence, and assembling it is itself a large part of the work nobody has time for. A worker produces its evidence as it goes.
For a given period, the record yields the population of actions, the subset that required approval, who approved each one, and the limits in force. That is the shape of most control testing.
The record cannot be quietly edited. Amendments are additive and attributed, so a changed record shows both the change and who made it.
